Step 1: Let’s see if you qualify!
- You are age 19+
- You have been living in Loudoun County for at least 3 months
- You do not have health insurance
- You do not qualify for Medicaid or Medicare
- You meet our income guidelines
Step 2: Fill out an application:
Step 3: Screening Phone call:
- After completing your application, our Eligibility Team will contact you within 5 business days to ask follow-up questions and determine your eligibility.
- In the meantime, we recommend gathering documents that provide proof of income and that
you live in Loudoun County. - Once you meet all eligibility requirements, you will be enrolled as a patient that day. You will
receive a membership card and schedule your first appointment
Step 4: What to expect in first appointment
- We ask that you arrive 15 minutes before your appointment to review and sign your new patient forms.
- You will then meet with a provider to discuss any questions or concerns you may have.
- After your appointment, you will receive a lab order for your free blood tests.
Step 5: Membership Renewal
Membership typically lasts for 11 months and can be renewed as long as you still meet the requirements.
